A Cancer Screening Patient Navigator is needed to support a population health program at CarePoint Health in Hudson County. Primary duties are to assist patients who identified as a high risk for developing lung and or colo-rectal cancer and overdue for cancer screening. The Navigator plays a pivotal role guiding the most at-risk patients, helping people prevent cancer, screen for cancer, and link patients to quality cancer treatment.
Under the supervision of the project director and system director cancer services, the Navigator provides individualized assistance to patients who are due for Lung/Colo-rectal cancer screening. Often the first point of patient contact; this position ensures that eligible patients receive lung and colorectal cancer screening, and follow-up with linkage to care. The Navigator needs to be self-motivated, driven by improving health outcomes and health equity, and with a solid ability to operate autonomously with guidance.
