ED Patient Navigator to assist with diagnosis of patients in need of testing for hepatitis C, hepatitis B & HIV   Grant funded position for a patient navigator (PN) to work in the emergency department (ED) at Hoboken University Hospital, using United States Preventive Services Task Force (USPSTF) guidelines, to identify and facilitate testing of patients meeting testing criteria for hepatitis C, hepatitis B and HIV.  The PN will work with ED staff to verify if patients meeting USPSTF criteria (age > 18) are asked if they have been tested for hepatitis C, hepatitis B and HIV.  For untested patients meeting criteria, the PN will work with ED staff to order testing.  The PN will also verify country of birth of patients seen in the Hoboken ED. Patients meeting demographic testing criteria for hepatitis B infection risk will have a serum hepatitis B surface antigen ordered.  The patient navigator will assist ED staff to use appropriate ICD-10 coding for testing.    The PN will work with the project director to review monthly reports of patients with a FIB4 score > 3.0 identified within Carepoint to identify those patients with advanced liver damage and screen them for hepatitis C, B and HIV.     The PN will work outpatient substance abuse services at Hoboken and inpatient substance abuse services at Christ Hospital to increase hepatitis C, Hepatitis B and HIV testing. The PN will maintain a list of patients tested and receive weekly reports of test results.  Patients testing positive will be referred to the appropriate infectious disease or liver clinic.  The PN will work with IT and the project director for compiling and maintaining data to transmit to the grant sponsor.

The Certified Nursing Assistant (CNA) under the supervision of a licensed nurse, the Nurse Assistant performs delegated tasks and procedures of patient care, involving the physical, psychological, social, emotional, safety, and security needs of the patient.    Essential Duties & Responsibilities: - Provides for activities of daily living by assisting with serving meals, feeding patients as necessary and ambulating, turning, and positioning patients; and providing fresh water and nourishment between meals. - Provides care by administering enemas, douches, non-sterile dressings, surgical preps, ice packs, heat treatments, sitz and therapeutic baths - Maintains patient stability by checking vital signs and weight; testing urine and recording intake and output information. - Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ call lights and requests; and reporting observations of the patient to nursing supervisor. - Handles the care and cleaning of specific equipment, stocking of supplies and equipment and the transportation of patients, supplies and equipment. - Any and all other duties as assigned

The licensed practical nurse is expected to perform tasks and responsibilities within the framework of nursing services and the scope of practice in a skilled nursing facility.  The LPN is part of the interprofessional team to help lead the resident and family teaching program through health teaching, health counseling, provision of supportive and restorative care, under the direction of a registered nurse or licensed clinician/provider. - Provide comprehensive care to patients by applying the nursing process in an organized and systematic manner according to policies of the Medical Center and The Transitional Care Unit in keeping with patient safety principles - Performs all clinical skills and services as per policy and accepted procedure within the LPN scope of practice - Completes clinical competency, required certifications, and peer evaluation process - Collaborates with the resident, significant others, ancillary departments, and functions as part of the multidisciplinary health care team whose goal is to provide care that is continuous and well-coordinated with specific discharge goals - Performs effectively and efficiently in emergency situations - Evaluates and documents resident progress/response toward expected outcome(s) and reports pertinent observations and recommendations to the interprofessional team - Communicates proactively to the interprofessional team the plan of care changes in patient condition, the patient progress, the discharge plan and other relevant information - Documents resident care and required safety checks accurately and legibly - Performs ongoing evaluation of residents’ physical and mental health and notifies the interprofessional team and/or clinician/physician, including abnormal lab and diagnostic results - Explains medications, procedures, and treatment protocols to residents - Administer medications via intravenous, oral, topical, subcutaneous, intramuscular, gastrostomy and other route of administrations within scope of practice - Administer enteral feedings via nasal gastric tube or gastrotomy tube - Perform bedside care such as wound dressing, changing bandages, inserting catheters, and giving injections - Obtain resident's vital signs such as respiration, temperature, blood pressure, and heart rate as needed and documented - Supervise certified nursing assistants and other auxiliary staff - Taking verbal orders from the provider/physician and document appropriately - Notifies physician of any pharmacist consultant’s recommendation for revision of medications, medication alerts, or changes in medication administration times as written in the Electronic Pharmacist Information Consultant (EPIC) form then implements the approved changes - Initiate and/or discontinue peripheral IV heplock - Call pharmacy for ordering prescriptions when discharging residents home - Provides hand-off report to receiving unit when transferring patients - Maintains a safe and comfortable environment for residents, visitors, and staff - Complies with policies and procedures concerning infection control and safety for all individuals on the unit - Maintains and develops competence through required in-service and continuing educational programs - Maintains current CPR and IV certifications - Discharge patients to home or other healthcare settings as supervised by a registered nurse - Active and contributing member of the Quality Assurance Performance Improvement Committee  

The Director of Grants is responsible for the financial reporting of grant related activities and for the overall management of grant funded departments. Leads grant preparation and submission process, oversees, and supports all aspects of the grant program, as well as conducting ongoing monitoring and evaluations. - Responsible for the day-to-day administrative management of all grants. - Communicate and work closely with CarePoint Health grants finance department, program directors and coordinators to develop and maintain all essential grant funded programs. - Schedule and lead bi-monthly meetings with program directors of all CPH funded departments to monitor contractual programmatic and fiscal compliance (includes monitoring of accruals and unspent funding). - Review and submit all grants applications and renewals. - Monitor all not-for-profit grants compliance requirements by funders. - Create and submit ongoing status updates and reports to funders, as required. - Tract all grants related expenses to ensure that they are allowable costs. - Engage regularly with funders to maintain strong working relationships. - Review and ensure that all not-for-profit policies and procedures are updated as required by Federal and State mandates. - Manages the day-to-day grants related financial requirements. - Responsible for management of grants related fiscal documentation, utilizing the QuickBooks entries (revenues, expenses, bill payments and monthly reconciliation). - Work closely with outside auditors yearly to complete required Uniform Guidance and single audits. - Assist with day-to-day operation of nonprofit entity and yearly fund-raising events.  

CarePoint Health Radiology is seeking to hire a part-time or full-time breast imager to join our practice.  The ideal candidate must be adept in all aspects of breast interventional procedures including ultrasound guided biopsies, stereotactic guided biopsies, and image guided needle localizations.  The ideal candidate will be fellowship trained and also clinically competent in general radiology.
