What You'll Be Doing

The Case Manager will work within an interdisciplinary team and collaborate with physicians, family members, nurses, and other hospital team members throughout the patient’s continuum of care. The Case Manager will provide compassionate quality care and concurrent assessment while assisting patients and families from pre-admission through discharge planning to ensure appropriate utilization of services in accordance with the Utilization Management Plan. 

 

   

     Performs utilization review function within 24  hours.

  • Formulates a plan of care to meet identified needs in most appropriate setting at time of initial assessment.
  • Performs utilization review function within 24 –48 hours of patient admission to the hospital and documents same on internal worksheet.
  • Interviews patient and/or family with risk criteria for education needs and any special discharge concerns and continues to follow through discharge.      

 

      Reviews plan of care with health care team and patient/family.

  • Attends interdisciplinary rounds and provides information to staff regarding Millman Care Guidelines, payor guidelines and identified variances in the care plan.
  • Identifies issues that require patient education prior to discharge.
  • Provides information regarding denials and LOS to medical staff.
  • Initiates communication with physician for clinical work plan.     

 

      Coordinates the plan of service that is identified for effective hospital stay and discharge planning. Works closely with the Social Workers and other members of the Interdisciplinary Care Team, to effect a timely discharge. This includes:

  • Assessment, developing a post-acute plan of care.
  • Reassessment as needed based on clinical needs, functional status, insurance coverage, caregiver support.
  • Implementation of safe and timely plan.
  • Makes referrals to members of the care team for post hospital care.
  • Coordinates hospital services such as diagnostic testing and treatment.
  • Identifies patient/family educational needs and makes appropriate referrals.
  • Performs retrospective and concurrent review by guidelines.

 

What We're Looking For

Full Time, Day Shift

8:30 am/9:00am - 4:30/5:00pm

Rotating weekends 

  • Prior case management experience
  • Diverse clinical experience in both Med/Surg and Critical Care a plus
  • 3 - 5 years clinical hospital experience required
  • Currently licensed NJ Registered nurse
  • Case Management Certification preferred
  • Excellent verbal and written communication skills required
  • Must be able to make decisions, work independently and communicate effectively with Interdisciplinary teams, patient, families, and physician staff to formulate a safe discharge plan
