What You'll Be Doing

The Licensed Clinician is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.

 

Makes psychosocial assessments appropriate to patient’s age and diagnoses.

  • Appropriately prioritizes cases according to assessed needs.
  • Written assessments completed within 24 hours of assessment.
  • Documented assessment demonstrates understanding of patient’s functional status and psychosocial situation.
  • Assessments are completed based on information obtained from patients, families/significant others, staff and interdisciplinary rounds as needed.
  • Assessment prepares for the possibility of changes in the patient’s condition/status.
  • Assessments are updated as new information is obtained.

Provides supportive counseling to patients and families

  • Demonstrates understanding and knowledge of counseling interventions through active participation in supervision.
  • Functions as liaison between family and treatment teams.
  • Interprets psychosocial information to members of the health care team.
  • Communicates psychosocial needs as appropriate for the continuum of care.
  • Utilizes group process to enhance coping skills.
  • Maintains knowledge base of current resources available and updates appropriately.

 

Coordinates case management services.

  • Collaborates with patient, family and other team members to coordinate services.
  • Serves as a resource for community services.
  • Provides assistance in obtaining alternative living situations when needed.
  • Works with health care team, patient/family and community agencies in order to facilitate efficient utilization of resources.
  • Develops and maintains good working relationships with health and social service agencies.
  • Provides timely documentation of discharge planning activities in the medical record.
  • Educates patients, family/significant others regarding options for discharge to include what is available and what has been arranged.

Documents appropriately

  • Documents comprehensive assessment findings and plan of care in chart, computer-based system and in interdisciplinary plan of care/critical pathway for all referred patients.
  • Additional duties as assigned. 

What We're Looking For

Full Time Day Shift Flex 

Monday thru Friday

  • Master’s Degree required in Counseling, Social Work, or Related Field
  • 2-4 yrs (required)

  • NJ LSW required; NJ LCADC preferred for all Addiction programs.
  • Valid NJ Driver’s License required for programs that include in-home visits
