JOB FUNCTIONS and DUTIES:
1. Responsible for the direct supervision of the clinical care managers and case managers which entails, but is not limited to assignment of work, transfer of work, probationary/annual performance reviews, competence assessment and job description revisions and updates.
2. Responsible for investigatory conferences, providing constructive criticism inclusive of formal discipline and recommendation for discharge along with participation in any grievance hearings resulting from discipline/discharge decisions.
3. Responsible for interviewing and recommending hiring of all new candidates within the case management department.
4. Provide direction and guidance to other clinical care managers, case managers and others regarding practice, serving as a resource and mentor.
5. Monitor the progress of medical social workers related to movement of patients along the continuum of care in accordance with regulatory standards with performance evaluation input.
6. Under general direction of the physician, provides health care in a variety of clinical settings within the scope of the Nurse Practice Act.
Knowledge of Federal, State Laws and Regulations, Accreditation and License requirements as governed by established policies and procedures.
7. Accountable to the physician for clinical care provided on a day-to-day basis; provides care in consultation with these physicians as appropriate.
8. Develop and implement a collaborative practice agreement specific to the clinical areas the Clinical Care Managers will conduct their practice within, and perform a periodic review of the collaborative agreement in accordance with hospital policy.
9. Maintain clear and accurate documentation according to hospital policy.
10. Obtain comprehensive health history, perform advanced physical examinations, write progress notes; prepare patient workups and summaries, noting pertinent positive and negative findings; assume responsibility for developing therapeutic relationships with patients.
11. Initiate admission orders with a special focus on Treatment Pathways.
12. Order and schedule laboratory studies and diagnostic procedures; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
13. Perform diagnostic and therapeutic procedures to assist physician in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care; record the normal and abnormal data for review by the physician.
14. Carry out therapeutic procedures such as starting intravenous lines, dressing changes; monitor progress of patients, report progress to attending physician, and maintain record of each patient’s progress, consulting with attending and consulting physicians when patient’s progress does not meet anticipated and / or predetermined criteria.
15. Prescribe and alter medication regimens, interpret findings and alter plan of care according to collaborative practice agreement and protocols in designated area.
16. Establish a diagnosis and order appropriate medical and nursing therapies.
17. Serve as a role model for the nursing staff in care delivery. Participate in identifying learning needs of staff. Function as a resource liaison and educator for the clinical caregivers.
18. Participate in performance improvement activities to meet the goals of the organization. Participate on committees as requested, and in interdisciplinary rounds using an integrated proactive approach to patient outcomes.
19. Review discharge needs of the patient and family. Facilitate post-hospitalization referral to assure appropriate discharge outcomes within designated time frames within the Clinical Care Manager’s role.
20. Educate patients and families on responsibility, management, and maintenance of care.
21. Maintain a basic knowledge of CMS and 3rd party coverage as necessary to facilitate patient care flow along the continuum, assuring continuity of care.
22. Assist organization in complying with regulatory requirements and standards.
23. Participate in development, implementation and monitoring of clinical pathways and standardized order sets.
24. Monitor the application of mandated performance measures as in but not limited to core measures. Correct any deficiencies concurrently to ensure 100% compliance.
25. Participate in the development, implementation, and evaluation of the goals of the Clinical Care Manager’s role, as requested by the assigned hospital collaborating physician.
26. Ensure that appropriate standards of practice follow corporate compliance guidelines, and that results of accreditation surveys and licensure reviews by NJDHSS and TJC are satisfactory.
27. Act in an ethical and legal manner, and assure that all actions are in the best interest of both patients and the hospital.
EDUCATION: Graduate of an accredited school of nursing, BSN, and Master’s degree in advanced clinical practice nursing or graduate of post Master’s certificate program for nurse practitioner.
EXPERIENCE: 1-2 years supervisory experience preferred. Two years full time as an Advanced Nurse Practitioner; five years preferred. Knowledge of healthcare procedures; Counseling/advising skills ; Decision-making and problem solving skills; Interpersonal/human relations skills ; Ability to maintain confidentiality ; Knowledge of patient care charts and patient histories; Knowledge of health education theory and practice; Ability to maintain quality, safety, and / or infection control standards; Knowledge of clinical operations and procedures; Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise; Knowledge of related accreditation and certification requirements; Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Working knowledge of Microsoft Office suite. Prior Meditech experience helpful.

LICENSURE / QUALIFICATIONS: Current NJ Licensure as a Registered Nurse; Certification as an Advanced Practice Nurse in specialty areas as required with prescriptive authority; PICC certified upon (3) three months of employment; BLS, ACLS, PALS certified(optional); Must be credentialed through Medical Staff Office.
WORKING CONDITIONS/HAZARDS: Exposure to communicable diseases, blood and blood products, illness, hazardous chemicals, X-rays and fluoroscopy, sharp instruments and needles, and electricity when in clinical areas.
PHYSICAL REQUIREMENTS: Includes standing, walking, moderate physical effort; lifting and maneuvering patients; occasionally must reach above head, carry medium to light equipment, and stoop.
