General Purpose of Position: TCMs provide system-wide coverage to patients of the Readmission Reduction Program, the Delivery System Reform Incentive Program (DSRIP), and the Bundled Payments for Care Improvement (BPCI) initiative. TCMs will drive the care management through direct oversight of care management through home visits, SNF on-site visits to patients from any of the three acute-care facilities and high-risk members of CarePoint Health.   Primary Responsibilities (include but are not limited to): - Responsible for the delivery of medical care service to a pre-designated group of Bundle Payment Care Improvement (BPCI) and DSRIP enrollees in collaboration with designated physicians. - Responsible for establishment and maintenance of effective external relationships with the BPCI and readmission reduction patients, key decision-makers, facility staff, and collaborating physicians.   - Responsible for establishment and maintenance of effective internal relationships with clinical and cross functional team members to ensure coordination of care across the continuum, physician engagement, patient engagement and patient activation. - Responsible for overseeing transitions of care including hospital, post-acute care, and home. PRIMARY CARE DELIVERY - Delivers cost-effective, quality care to assigned BPCI patients in collaboration with their assigned physicians. - Manages both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty provider. - Assists to develop and implement clinical plans of care for patients with chronic and terminal illnesses - Performs a wide range of activities, which include but are not limited to: assessment, history and physical, and recommendations for aggressive symptom management. - Develops plans to meet patient and family physical and psychosocial needs. - Monitors patient progress, regularly updates the team through designated paper and electronic documentation protocols; regularly updates the primary care physician; consults regarding discharge planning; and coordinates patient/family teaching. - Performs comprehensive assessments and documents findings in a concise/comprehensive manner that is compliant with BPCI and readmission reduction program visits and documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations. - Maintains patient medical records according to established standards and documents in a systematic, accurate, and succinct format. - Facilitates agreement and implementation of each patient’s plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians while ensuring compliance. - Maintains communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis. - Refers patients to Physician or to specialized healthcare resources for consultation or treatment as appropriate. - Initiates communication with attending physicians, other transitions in care staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family. - Evaluates the effectiveness, necessity, and efficiency of the plan of care and makes revisions as needed. - Utilizes practice guidelines and protocols established by CarePoint in compliance with the BPCI program. - Promotes the development of a collegial team (for coverage, trouble shooting, and brain storming.) - Fosters and develops a culture of clinical expertise. - Serves as a role model to internal and external partners. - Required to participate in on-call program. - Required to do home and post-acute care visits. - Required to do PICC line insertion as indicated or assigned. CARE COORDINATION - Understands the BPCI and readmission reduction programs, including associated policies and procedures, and articulates them effectively to providers, members, and key decision-makers. - Assesses the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of services and communicates process for providers in accordance with BPCI and readmission reduction expectations. - Coordinates care as patients transition through different levels of care and care settings per the care transitions protocol. - Ongoing monitoring of needs of patients and families while facilitating any adjustments to the care plan as situations and conditions change. - Orders and interventions are reviewed for appropriateness and response to treatment to identify most effective plan of care in alignment with the member's needs and wishes. - Evaluates plan of care for cost effective services that provide agreed upon outcomes for patients, families and providers and decreases high costs, poor outcomes and unnecessary hospitalizations. - Activates additional home visits and/or transitional care phone calls based on patient’s progress and clinical presentation. - Facilitates TCU admission or home visits as is appropriate for patients presenting to the emergency room with conditions that can be appropriately managed outside of the inpatient setting. - Transition coaching, educating, coordination in collaboration with the team RN, team medical assistant, CarePoint hospital staff, CarePoint CMOs, and the patient’s PCP. - Case Management of all BPCI patients for 90 days post discharge from the hospital; including coordination with facility staff, home care agencies, SNF and TCU facilities, and PCPs. PROGRAM ENHANCEMENTS AND EXPECTED BEHAVIORS - Regular and effective communication with internal and external parties including physicians, members, key decision-makers, nursing facilities, Transitions in Care staff and other provider groups. - Actively promotes the BPCI and readmission reduction program in assigned facilities by partnering with key stakeholders (ie: internal sales function, provider relations, facility leader) to maintain and develop program growth. - Exhibits original thinking and creativity in the development of new and improved methods and approaches to concerns/issues. - Functions independently and responsibly with minimal need for supervision. - Demonstrates initiative in achieving individual, team, and organizational goals and objectives. - Participates in BPCI and readmission reduction quality initiatives. - Participates in orientation and ongoing education programs. - Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services. - Other projects and duties as assigned.
